If you would like to have a transcript sent to an educational institution, please click here to email the information below.

  1. Your complete name (maiden also, if applicable)
  2. Date of birth
  3. Date of graduation or year that you left LHS
  4. Name and address of institution to receive transcripts

If you would like to have a transcript sent to an individual, your signature is required. Click here to download the form that may be faxed or mailed to the address shown on the form. This is a PDF document and requires Acrobat Reader to open.

Note: Parents may not request transcripts for students 18 years or older.
